This is regarding the necessary documents to be attached in death claim case file for waiver of succession certificate.

Regions are requested to check whether the following documents are attached in the death claim case file before sending it to the circle office to avoid any delay in processing   the case.

Ø  ORIGINAL POLICY BOND

Ø  INSURANT DEATH CERTIFICATE

Ø  INSURANT CAUSE OF DEATH CERTIFICATE (MEDICAL CERTIFICATE (or) VAO CERTIFICATE)

Ø  NOMINEE DEATH CERTIFICATE

Ø  LEGAL HEIR CERTIFICATE

Ø  CLAIMANT REPRESENTATION LETTER

Ø  CLAIM APPLICATION FORM

Ø  CLAIMANT ID PROOF

Ø  AFFIDAVIT (PLI Dte’s letter No. 25-01/cif/2012-LI (Vol.I) dated 07.01.2019 )

Ø  LETTER OF INDEMNITY (PLI Dte’s letter No. 25-01/cif/2012-LI (Vol.I) dated 07.01.2019 )

Ø  LETTER OF DISCLAIMER (PLI Dte’s letter No. 25-01/cif/2012-LI (Vol.I) dated 07.01.2019 )

Ø  AFFIDAVIT (PLI Dte’s letter No. 25-6/2020-Li dated 21.06.2021 )

Ø  AUTHENTICATED CALCULATION SHEET

 

Regions are also requested to make sure that the recommendation letter from the regional office should have the sentence,

 " The case file is submitted to circle office duly recommended by PMG, ______ Region. "
